SWL Integrated Care Project Showcase
Clinicians and local commissioners from south west London came together on Monday 28 April to showcase local projects and initiatives funded by Health Education South London (HESL).
The event was the culmination of work over the past 5 months which has sought to support workforce development and integration through a number of initiatives, including Multi-Disciplinary Team working across health & social care.
HESL’s Director of Education & Quality and GP Dr Nav Chana, presented at the event and described it as a great opportunity to share and learn from best practice and success. “These initiatives– developing the workforce to integrate care around patients – are key to developing a system wide response to some of the population healthcare challenges we face”
Melanie Ashdown, Programme Manager for SWLCC said “It is only through strengthened partnership working with our CCGs, Local Authorities and education commissioners and providers that we are able to facilitate change at grass-roots level”
Projects included: Nursing Home Pathway Training (Croydon CCG), Patient Assessment Skills for Community Based Nurses (Croydon CCG), Development of Primary Care led MDTs and Case Management (Kingston CCG and Croydon CCG), Practice Based MDTS, Health Coaching, Practitioner Forums (Merton CCG) and Intermediate Care Services and Re-ablement Team (Richmond CCG)
