Health and Care Events 2018
The South West London STP refresh and discussion document which was published in November 2017 made it clear that we would be working in local partnerships at borough level to integrate health and care, and encouraging partnerships to develop local health and care plans.
Since November 2017 local partnerships have been reviewing their borough population’s health needs, current services, listening to local people, to identify and understand shared health and care priorities to work together to support people in each borough to start well, live well and age well.
Each local partnership have hosted a Health and Care event in their borough to engage on their Local Health and Care Plan.
Each event brought together around 150 people
including partners, stakeholders, front-line staff and the local community to help us focus on the actions we are going to take within each boroughs local Health and Care plan.
The Health and Care events are part of a bigger piece of continuous engagement on local health and care priorities – as we are all already talking and will continue talking to key people about working together.
Local health and care plans for each of our boroughs will be published in March 2019. These plans will focus on how we can work in partnership to meet the challenges we have set out in the discussion document, and provide the best care for people with complex needs. We will continue to work together with local people, community organisations and our partners to put these plans into action in the months and years to come.
Health and Care plan events were held in each borough on the following dates:
- Richmond 19th November
- Croydon 20th November
- Merton 21st November
- Kingston 21st November
- Wandsworth 28th November
- Sutton 29th November
