Transforming Care Partnership
Background
We’ve been supporting the Transforming Care Partnership team to engage with people around the plan to improve services for people with learning disabilities and or/autism.
The Transforming Care plan aims to reshape services for people with learning disabilities and/or autism by reducing some inpatient provision and strengthening the support available in the community. This vision is set out in “Building the Right Support”.
In 2016, the Transforming Care Partnership team produced the south west London TCP plan. The plan outlines how the partnership will ensure that person centered care is at the heart of services for people with learning disabilities and/or autism.
Transforming Care Partnerships were set up across England to take forward the plans. They are a collaboration of CCGs, local authorities and NHS England specialised commissioners. They bring together commissioners responsible for funding health and social care for people with a learning disability and/or autism.
Engagement
To ensure that we achieved meaningful engagement and involvement from children, young people and adults with a learning disability and/or autism and the families/carers and those voluntary and community groups that support them we developed a package of engagement
- Engagement directly with service users, carers and families – currently being delivered by Paradigm
- Meeting with and listening to each of the advocacy groups in south west London who support people with learning disabilities.
We met with each advocacy group and asked for their advice on where and how we should speak to people with learning disabilities and their carers about the plans.

The team have also produced a midpoint review in easy read which outlines the progress made from 2016 – 2017.
